Henry Ford Hospital

Henry Ford Hospital in Michigan experienced the loss of health data that affected 2,777 individuals.

Cancer Care Northwest

The loss of healthcare information in paper form affected 3,100 individuals at Cancer Care Northwest, located in Washington.

Charleston Area Medical Center

The West Virginia-based health system experienced the unauthorized access/disclosure of health information stored on a network server, affecting 3,655 individuals.

MidState Medical Center

Healthcare information stored on hard drive was lost at MidState Medical Center in Connecticut, affecting 93,500 individuals. Some 500,000 individuals were affected when a hard drive went missing from Blue Cross Blue Shield of Tennessee in 2010.

Reid Hospital

Laptop theft at Indiana-based Reid Hospital affected 22,001 individuals. University of California, San Francisco and City of Hope National Medical Center were two organizations hit by laptop theft last year.
