These 10 hospitals scored an 'F', pose greatest risk to patients, Leapfrog says
As 1,000 patients a day still die from preventable errors, Leapfrog report shows which facilities are failing to improve.
The Leapfrog Group on Wednesday released its spring Hospital Safety Grade, a biannual program that grades U.S. hospitals on how well they prevent errors, accidents and infections.
Leapfrog rated 2,639 hospitals, and 823 hospitals earned an A, 706 earned a B, 933 earned a C, and 167 earned a D.

But the following 10 hospitals were the only ones to receive an F:
