Healthcare IT News and HIMSS are accepting topic and speaker proposals for Pop Health Forum 2016 scheduled for September 12-13 in Chicago.

The May Pop Health Forum in Boston attracted nearly 250 attendees and included keynotes by leaders from Geisinger Health System and Intermountain Healthcare, as well as 30-plus other pop health experts from around the United States. The Chicago event will include a similar line-up of speakers.

This event is how-to focused and intended for healthcare payers and providers. Case studies are a priority. This is a great opportunity to share an organization’s new program, success story, and/or lessons learned and to network with like-minded professionals.

Proposals from vendors are not accepted.

Proposals should address one or more of the three cornerstones of population health:

  • Data and Analytics
  • Care Coordination
  • Patient Engagement and Experience

Other topics relevant to achieving population health also will be considered.
