Saint Luke's Health System Goes Live With Covisint As It Seeks PCMH Designation

Kansas City-based Saint Luke's Health System is now using Covisint healthcare to help Saint Luke's achieve its patient-centered medical home (PCMH) designation. The cloud engagement platform from Covisint, is helping Saint Luke's improve care delivery by establishing key population metrics.

Healthcare reform is increasingly making the industry's primary stakeholders--physicians, hospitals, patients, families and payers--jointly accountable for the cost and quality of care. As an 11-hospital, 300-physician health system based in the Kansas City area, Saint Luke's chose Covisint to help proactively improve patient care and manage population health.

Specifically, Covisint's cloud engagement platform is helping Saint Luke's physicians:

• track and report on care delivery throughout the population;
• understand where their most critical care needs are; and,
• develop system-wide strategies to improve the community's health.
