Post-acute facility trims length of stay by 3-5 days with alerts tech

Since implementing the alerts technology, Cedar View has trimmed its average length of stay for Medicare Advantage patients by three to five days.
“The biggest reason for this improvement is that now we can obtain the contact information of the external case manager before the patient is admitted to our facility,” Gele said. “As a result, we’re better prepared to engage with the patient on Day One.”
Cedar View also has seen a 6 percent decrease in readmissions because it has been able to coordinate care better with external providers. At the same time, physicians and medical teams now are more engaged in Cedar View’s transition of care program.

Moreover, with improved access to contact information for hospital case managers and treating providers, skilled nursing facilities can function better within the patient’s overall care team.
“Skilled nursing facilities and home care agencies can use the same technology to alert other providers when they admit or discharge patients,” Gele explained. “It’s a win for everyone concerned, especially the patients.”
