We want your population health story.

Just a few days remain to submit speaker and session proposals for the Healthcare IT News and HIMSS Pop Health Forum 2016 which will be held in Chicago, Sept. 12-13.

The deadline for submitting a proposal has been extended to 5 p.m., June 23.

Speaking opportunities are limited to professionals and experts from healthcare provider and payer organizations, government agencies and academic institutions. Presentations should be practical, actionable and solutions-based. Case studies are a priority for this event.

Driven by the value-based care and other new reimbursement models, population health is one of the hottest and most important topics in healthcare. Many provider organizations don’t know where to begin or, if they have begun, how to demonstrate ROI (improved care, reduced cost, higher patient satisfaction) and sustain their efforts.

As such, we place a high value on proposals from provider and payer organizations that offer practical, actionable information and real-life solutions. Case studies are a priority for this event.

The two-day Pop Health Forum 2016 will bring together more than 250 leading providers, payers, researchers and government officials.
