Caradigm debuts risk stratification tool for care management workflows

The population health IT vendor says streamlined enrollment into care management should improve team efficiency and initiation of care.

Population health IT vendor Caradigm today unveiled at HIMSS17 the Caradigm Risk Stratification system, a patient list application that’s integrated with the vendor’s suite of systems including Caradigm Care Management, an enterprise care coordination workflow application.

Because the new risk stratification system is integrated with the Caradigm suite, provider organizations using the suite can realize a variety of benefits when looking to identify and manage the care of patients in value-based care programs, Caradigm said.

The risk stratification tool, for example, can aggregate data from multiple EHRs in real time, create segmented patient lists using clinical indicators, identify patients that meet defined criteria, and streamline enrollment into programs within the care management tool so that care teams can engage patients more quickly to drive better outcomes, the vendor explained.

Caradigm said that the risk stratification tool can be implemented rapidly as an add-on to already deployed Caradigm systems. The application also includes a standard set of out-of-the box lists, and can support the creation of custom lists, with plans to plug in third-party predictive risk algorithms, Caradigm added.
