3M unveils new 360 Encompass MD tool, for CMS Hierarchical Condition Codes, at HFMA ANI

Patient Insights Suite analyzes EHR and claims data to identify HCC patients and make care recommendations to improve patient outcomes and population health, say company officials.

3M Health Information Systems has added another physician workflow and analytics tool to its 360 Encompass MD repertoire with its newest tool: Patient Insights Suite. It was announced today at HFMA ANI in Las Vegas.

Patient Insights analyzes, documents, codes and reports data to accurately assign hierarchical condition categories with an interoperable workflow, according to 3M – including advanced revenue applications. Providers can then identify patients with missing HCCs, receive guidance based on HCC diagnosis and understand HCC risk factors.

The platform is part of 3M 360 Encompass MD system, which is a physician workflow technology that integrates will all major EHR systems to identify gaps in documentation.

"With the healthcare industry's transition to value-based payment, many organizations are hiring additional staff and performing costly manual chart reviews in order to clarify health conditions of their patient populations," said James Lawson, vice president of payment and population solutions at 3M Health Information Systems, in a statement.

"With Patient Insights, healthcare providers can instead automatically identify high-risk patients or analyze high-cost encounters and quickly take action to impact care quality and protect the bottom line," said Lawson.

Patient Insights integrates EHR and claims data to create a complete profile of a patient's health condition. The platform addresses potential care gaps and continuous high-cost care areas, while identifying high-risk patients with real-time data. According to the announcement, the platform improves care outcomes, population health and provides more accurate reimbursement.

3M will release another Patient Insights tool in the fourth quarter of 2016, which will include care management functions for daily workflow, customized patient worklists based on risk analysis, communication tools and a detail reporting module.
