Xerox launches population health platform
LAS VEGAS -- Xerox launched a new population health management solution at HIMSS16. Dubbed Xerox Health Outcome Solutions, the tool can be used to help at-risk organizations better prepare for the new reality of bundled payments and accountable care.
Most healthcare professionals know by now that the Centers for Medicare and Medicaid Services plans to require 50 percent of payments through value-based arrangements by 2018. But "from our research, we found providers are having trouble getting their heads around this," says Tamara StClaire, chief innovation officer of Xerox Healthcare. "In this growing market, providers are really struggling with underperforming value-based contracts. There's an opportunity to identify specific areas of improvement."

Pointing to research showing 69 percent of health organizations uncomfortable with the risks of value-based reimbursement – and 77 percent aware of the very real prospect of losing money with this new approach – Xerox has rolled together a suite analytics, clinical and administrative tools that can help providers tackle readmissions and other key quality goals.
"We take a lifecycle approach," StClaire said. "What's unique about the way we're going to market is we can take it from soup to nuts."
The Health Outcome Solutions platform combines analytics from Xerox’s Care Integration Services, which can assess defined patient populations and identify those most at risk of readmissions; clinical tools such as those offered through the Juvo Care Performance Platform developed by its subsidiary Midas+; other IT infrastructure (cloud hosting, system integration, claims eligibility and submission tools) to enable telehealth services, and administrative technology for appointment scheduling and reminders, patient onboarding and more.
StClaire said that type of holistic "actuarial approach," where Xerox is "able to get our head around the at-risk components of their contracts, understand the population cohort that's attributed to them" – and then help providers "prioritize programs that are going to move the needle," is a key part of the new offering.
"We're not just offering a population health program that we're going to try to sell, we're going to take a step back and do some much needed analysis with the provider what programs need to be put in place," she said. "We can provide customized solutions that are going to provide an ROI and help folks with those value-based contracts."
